Registration


We have tried to simplify the registration process for parents. To achieve our goal, we ask that parents wishing to register their children in any one or more of the programs open for registration follow the directions below.

The  forms essential to registration in all programs are:
1. 2012 Participant Information Form for Special Needs Programs (which is completed and submitted once in a calendar year)

2. 2012 Daisy Association Membership Form  (You must have current membership in the Daisy Association during the time your child participates in any programs during the calendar year. If a program begins in one calendar year and continues into the next year, you must continue your membership by paying the $25.00 yearly dues and completing the Daisy Association Membership Form for the new year)

3. Program Registration Form (which must be completed and submitted for registration) This form will be updated generally once every three months to reflect any programs that will have registration available beginning in that month. It may have more than one program listed. All you will have to do is complete the information on that form, place a check mark next to the program(s)being selected, read the check list steps, download any additional forms or information that may be required to participate in any specific program and send all the registration material and check(s) to the Division of Recreation before the deadline date listed for each program.

4. Participant Profile (which is completed and submitted once in a calendar year)

5. Policies and Procedures This is information relative to registering in and participating in any of our special needs programs. You are not required to sign and return it but are required to read it, and if you wish, download it. On the 2012 Participant Information Form for Special Needs Programs, you are asked to indicate that you have read the Policies and Procedures.

6. Frequently Asked Questions This is information relative to registering in and participating in any of our special needs programs. First, open the current Program Registration Form and see what programs are
available. If one or more programs listed are the ones you are interested in having your child join, then download the Program Registration Form. Second, download the 2012 Participant Information Form, (only submitted once in each calendar year.). Third, download the Participant Profile (only submitted once in each calendar year.). Fourth, download the 2012 Daisy Association Membership Form (only submitted once in each calendar year.) Fifth, download the appropriate Check List for that program.  Sixth, download any additional forms listed on the Check List that are needed.
